Neutropenic enterocolitis in a non-immunocompromised patient presenting with lower respiratory tract infection: a case report

Neutropenic enterocolitis (NEC) has been widely reported in adults with myelofibrotic disorders, solid malignant tumors, bone marrow transplantation as well myelotoxic chemotherapy. Here we a case of 66-year-old non-diabetic, hypertensive female without any prior history immunosuppression or underlying malignancy related chemotherapy; who presented severe dyspnoea, low grade fever and cough. Followed by course effective conservative management initial stabilization, on investigation her TLC was found to be abnormally neutropenia hyperinflammatory response induced myelosuppression LRTI suspected. Subsequently, subcutaneous G-CSF, broad spectrum antibiotics were initiated along supportive management. She developed abdominal distension, diffused pain tenderness hypoactive bowel the following day. After 3 days G-CSF trial, neutrophil count started increase overall improvement. CECT performed positive findings multiple colonic short segment involvement stricture, pericolic fat stranding perforation; reflecting towards resolving neutropenic enteropathy.
